Transition from pediatric care to adult services is a normal transition for all adolescents; however for youth with spinal cord injury or other special health care needs, this transition requires special attention. Proactive management should begin early and address expectations of both caretakers and the individual youth. The transition process should include not only health management and transfer to adult care but also education, vocational planning, independent living, and psycho-emotional health. Individuals need to be viewed holistically with recognition that their health and quality of life can be positively enhanced by a successful transition from adolescence into adulthood.
