Life care planning is a case management tool utilized by rehabilitation counselors, rehabilitation nurses, physiatrists, rehabilitation psychologists, and other health care professionals who have assumed the role of the board eligible or board-certified case manager in implementing/overseeing rehabilitation programs and support care services for the catastrophically disabled. It is based on specific standards and methodologies to which practitioners are expected to adhere. The history of life care planning goes back to the 1970s, with the first publication in the field in 1981. Currently, several rehabilitation counseling, nursing, and disability examining boards are involved in oversight related to life care planning.
